Appetite Questionnaire

Before you begin, we would like to thank you for taking time to complete this questionnaire. The anonymised data collected from this questionnaire will be used in research aiming to understand poor appetite in older adults. This questionnaire should take approximately 5-10 minutes to complete. At the end of the questionnaire, you can indicate if you are happy to be contacted to take part in an interview to talk in more depth about your experience of poor appetite.

The questions appear in sections, please ensure you complete all the sections. It is up to you if you wish to participate. You can withdraw from this questionnaire at any time simply by closing the browser web page. By submitting your responses to the questionnaire, you are providing your consent to participate in this questionnaire and for the researchers to access your responses. All data will be anonymised.
